MEDICATION PURCHASE AND CANCELLATION POLICY
_____Payment: Payment in full must be received on or before the first day of your telemedicine appointment. We offer the following payment options:
_____Credit Cards: We accept Visa, MasterCard, American Express and Discover.
_____Punctuality: Please be prepared 5 minutes before appointment time so you can prepare for your telemedicine experience and enjoy your treatment. Arriving late: By arriving late, you will disrupt your treatment, reducing the time available for the treatment. Your treatment will end at your scheduled time and there will be no disruption with the next appointment.
_____No show: We recommend that you get in touch with us 24 hours before your scheduled appointment to let us know if you will be late. No shows lead to the disengagement or voiding of any agreements you may have with our office. Should you fail to arrive for your scheduled appointment without notifying us in advance, your deposit or future appointments may be forfeited.
_____Full Vial Single Payment: This purchase is a one-time transaction with no additional commitments. By completing this purchase, you agree that you are not obligated to make any further payments or purchases in the future.
All sales are final, and no recurring charges will be applied to your account. Please review your order carefully before finalizing the transaction.

_____Recurring Monthly Payment: By purchasing this item, you acknowledge and agree to a recurring monthly payment structure. If you decide to discontinue the service, you will remain responsible for the agreed-upon initial vial cost and any remaining balance determined by your weekly dosing volume of the medication.
Please note that the weekly dosing may vary based on your individual results and ongoing collaboration with Reveal Physique. It is essential to maintain clear communication with our team regarding your dosing adjustments and any concerns you may have about the service.

_____Cancellation: We adhere to a strict one-week cancellation/rescheduling policy. In the event you fail to reschedule your appointment one week prior to the initial appointment, cancel your appointment, or do not appear, you may forfeit your future appointments as well as any unused money or deposits. An appointment cancelled 8 or more days before it is normally scheduled will result in a 5% loss of all fees to cover credit card charges. There will be a 25% fee loss if you cancel four to seven days prior to your scheduled appointment time. If you cancel less than 72 hours before your scheduled appointment date, you will be charged 50% of the total fee.
After the initial treatment, no refunds are allowed.
_____RETURN POLICY
Reveal Physique is a medical clinic providing the latest safe and effective treatments for weight loss. Reveal Physique is a cash clinic that does not accept medical insurance. We provide full paperwork for patients to seek reimbursement for any services and prescription medications that may be covered by the patient’s insurance. Payment for services is the responsibility of the patient regardless of insurance coverage.
This return policy constitutes the only conditions applying to the return of products and services, no other policy to this effect exists, and this policy supersedes all prior policies.
_____GENERAL:
Reveal Physique does not offer refunds on accounts with outstanding claims or balances; or where upcoming services are scheduled.
Reveal Physique does not allow the transfer of services or treatments to other individuals or other clinics.
Submission of the returned product does not constitute acceptance of the return. Reveal Physique reserves the sole right to determine whether the products or services qualify for replacement/refund. Reveal Physique also reserves the sole right to refuse a return as it deems appropriate.
Refunds less than $25.00 will not be processed.
_____ITEMS DEEMED RETURNABLE
Reveal Physique will not issue a refund, but will promptly replace items meeting the following criteria:
1. Recalled products.
2. Incorrect shipments
3. Damaged product due to shipping carrier
_____ITEMS DEEMED NON-RETURNABLE
Reveal Physique does not accept the return or refund of:
Services provided or performed; including but not limited to physician visits and patient training. If, initial patient training was unsuccessful. Reveal Physique will provide two (2) additional trainings at no cost to the patient.
Products or services not purchased directly from us. The company reserves the right to destroy non-company products shipped as a returned product and to charge the patient for the cost incurred to process and destroy the non-company product.
Prescription medications, including but not limited to any prescription written by a licensed physician.
Damaged products, including but not limited to missing or damaged labels, repackaged product, opened product, and product otherwise not in its original form.
Mishandled products, due to patient's improper handling or storage of the products.
_____PROCESS FOR RETURNS/REFUNDS
Reveal Physique will only accept the return of product for consideration of refund, if applicable, under the following conditions:
1. For recalled products:
In the event of a recall initiated by the pharmacy, or any other pharmacies used by Reveal Physique, or government agency, Reveal Physique agrees to pay reasonable out-of-pocket costs for costs incurred by the patient.
Reveal Physique will provide the patient with instructions on how to return any recalled product.
2. For incorrect shipments, and products damaged due to shipping carrier:
Reveal Physique must be notified with in seven (7) days of the incorrect or damaged shipment. Please contact the phone number on the website that you signed up for the service or in the header of this form.
Mail the product back to us via USPS to addresses listed on the website that you signed up for the service or the address listed in the header of this form
Damaged products will be replaced, not refunded, within seven (7) business days after receipt of an approved return.
_____CANCELLATION OF FUTURE PURCHASES
Our company maintains a strict policy regarding refunds for discontinued services initiated by customers on monthly payment plans. In the event of cancellation, any additional charges necessary to settle the outstanding balance will be applied. We appreciate your understanding as we strive to uphold these terms to ensure a smooth and fair process for all our customers.
_____CHARGEBACKS INITIATED BY THE PATIENT
Reveal Physique takes patient care very seriously, and our clinics strive to provide the highest quality care to every individual. In the event the patient is not satisfied, we ask that you please refer to this document rather than initiating a chargeback with his or her credit card company. If a chargeback is initiated, Reveal Physique will use all documentation available to refute the chargeback including patient acknowledgment of the refund policy.
